COVID-19 Self-Reporting Form

DTS uses this self-reporting form to assist in contact tracing and provide information to those who are COVID-19 positive or exposed. The following categories determine who should complete the form and what important next steps should be taken.
Unvaccinated: Exposed
Unvaccinated individuals exposed to or living with someone who is COVID-19 positive must:
- Begin five (5) day self-isolation.
- Complete the DTS self-reporting form below.
- Get tested for the virus 72 hours after exposure or self-isolate for five (5) days (no COVID-19 test needed).
- Complete self-reporting form if COVID-19-positive.
- Follow the instructions that DTS will send as a result of the self-reporting form.
Unvaccinated: Positive or Symptomatic
Unvaccinated individuals who test positive for or exhibit symptoms of COVID-19 must:
- Begin five (5) day self-isolation.
- Complete the DTS self-reporting form below.
- Get tested for COVID-19 and/or see a physician.
- Wait for results.
- Follow the instructions that DTS will send as a result of the self-reporting form.
Vaccinated: Exposed
Vaccinated individuals exposed to or living with someone who is COVID-19 positive do not need to self-isolate or complete the self-reporting form unless they test positive or are symptomatic.
Vaccinated: Positive or Symptomatic
Vaccinated individuals who test positive for or exhibit symptoms of COVID-19 must:
- Begin five (5) day self-isolation.
- Complete the DTS self-reporting form below.
- Get tested for COVID-19 and/or see a physician.
- Wait for results.
- Follow the instructions that DTS will send as a result of the self-reporting form.
DTS no longer requests that employees obtain a negative COVID-19 test result before returning to campus.
